Preventing, assessing and treating skin tears
This module looks at the characteristics of skin tears, details the risk factors for skin tears and explains the importance of using a holistic, evidence-based and person-centred approach to skin tear prevention, assessment and treatment. It also outlines the crucial role of nurses as part of the multidisciplinary team in the management of skin tears. Learning aims and intended outcomes After completing this module and the time out activities you should be able to: Describe the main risk factors for skin tears and identify patients at risk of sustaining a skin tear. Outline the principles of a skin tear risk reduction programme. Assess a skin tear. Choose the appropriate dressing for various types of skin tears. Explain the importance of holistic, evidence-based and person-centred care in the prevention, assessment and treatment of skin tears.
